Transition Services

Transition services are those services which enable a student with disabilities to progress to another stage in life. Transitioning through these stages (such as moving from elementary school to middle school) can be difficult for most students, especially those with disabilities.

We offer coordinated services for students with disabilities to help them meet their transition goals. SKACD works collaboratively with community agencies to provide support to our students’ during the transition process. A few resources are provided here.
